Gale McCombie was sitting in a hospital waiting room when she spotted a brochure offering free yoga classes for cancer patients.

But zapped of strength and confidence after undergoing grueling treatments for breast cancer, McCombie found the thought of entering any exercise class intimidating.

"Chemo left me in a pretty bad state as far as my physical ability. But everybody talked about the importance of physical activity, and yoga seemed to be a good start," the 50-year-old Canadian said in an interview

"I took my mom with me for comfort. I didn't know what to expect. But the others had cancer, too. And the facilitator made sure everyone was comfortable with props, modifications, whatever it took. After the first class I was fine."

McCombie joined Yoga Thrive, a community-based program created specifically for cancer survivors that is spreading throughout Canada and parts of the United States.

The brainchild of Dr. Nicole Culos-Reed, a cancer and exercise researcher at the University of Calgary, and Susi Hately, a yoga instructor, Yoga Thrive provides gentle, therapeutic yoga to people at all stages of cancer treatment.

No Personality Lnk to Breast Cancer

Women who were unemotional, depressed or anxious were no more or less likely to get breast cancer than any other women, the study found. Nor were women who were optimistic, angry or understanding, or had any combination of personality traits.

The Dutch researchers measured 11 personality traits in 9 705 Dutch women in a survey in 1989 and 1990, then tracked them through 2003 to see who got breast cancer. "The present results indicate that women should not worry about a possible impact of personality factors as a risk factor for breast cancer," Eveline Bleiker of the Netherlands Cancer Institute and Antoni van Leeuwenhoek Hospital in Amsterdam, who led the study, said by e-mail.

"Moreover, women with breast cancer should not worry that their character might have contributed to the development of their disease," Bleiker added. Bleiker noted that some researchers in the 1980’s advanced the idea of a "cancer-prone" personality with such traits as stoicism and difficulty in expressing emotions.

"The earlier studies had a number of methodological flaws," Bleiker said, adding that more recent, large studies have not found the existence of a "cancer-prone" personality. "In summary, there is currently no convincing evidence that such a personality profile exists," Bleiker said.

In 1996, Bleiker's team studied the same group of women for five years and found that one of the 11 personality traits, what they called "anti-emotionality", actually was associated with a slight increased risk for breast cancer. But after tracking the women for a longer time in the new study, this association vanished. This suggests it may have been only a chance finding, Bleiker said. Women with the "anti-emotionality" trait responded negatively to survey questions such as: "In important situations, I trust my feelings," "I respond emotionally to people," and "My behaviour is influenced by my emotions."

Kevin Stein, a researcher at the American Cancer Society who was not involved in the Dutch study, said it is inappropriate and factually incorrect to blame a woman's personality for her breast cancer. "I am pleased to see this stuff knocked down," Stein said in a telephone interview. "There's really no support for the idea that your personality or your attitude can either cause a cancer or prevent or help avoid a cancer."

According to US research, a dangerous type of breast cancer, known as inflammatory breast cancer, was seen in 45% of obese patients, compared with 30% of overweight patients and 15% of patients of healthy weight.

"The more obese a patient is, the more aggressive the disease," said Dr. Massimo Cristofanilli of the University of Texas M.D. Anderson Cancer Center, who led the study.

"We are learning that the fat tissue may increase inflammation that leads to more aggressive disease."

Writing in the journal Clinical Cancer Research, Cristofanilli and colleagues said they studied 606 women with breast cancer that had spread within the breast.

They classified them according to body mass index or BMI, a globally accepted measure of obesity. People with BMIs of below 25 are considered normal, while 25-29 marks overweight and 30 or above is clinically obese.

After five years, 56.8% of obese women and 56.3% of overweight women were still alive. But 67.4% of the normal weight women had survived.

More than 56% of women of normal weight survived 10 years, compared to 42.7% of obese women and 41.8% of overweight women.

"Obesity goes far beyond just how a person looks or any physical strain from carrying around extra weight. Particular attention should be paid to our overweight patients," Cristofanilli said.

Many studies have shown that the obese have a greater risk of several types of cancer. Last month British researchers reported in the Lancet medical journal that obesity can double the risk of leukemia, multiple myeloma, thyroid cancer, colon and kidney cancers.

Fat cells produce a range of hormones that could fuel cancer, researchers say.

Exercise Cuts Breast Cancer Risk

According to researchers for the Journal of the National Cancer Institute, women who were the most physically active were 23% less likely to develop premenopausal breast cancer than the women who got the least exercise.

High levels of exercise from ages 12 through 22 contributed the most to the protective effect, the researchers said.

"The more activity, the greater the benefit," said study leader Dr. Graham Colditz of Washington University School of Medicine and Barnes-Jewish Hospital in St. Louis

Previous studies showed that regular exercise in adulthood leads to at least a 20% lower risk of breast cancer after menopause. But previous research on exercise and breast cancer risk before menopause had produced inconsistent results.

The researchers said the new study indicated women need regular physical activity starting at a young age to comparably lower their risk of breast cancer before menopause. They called their study the largest and most detailed examination to date of the impact of exercise on early breast cancer risk.

Colditz and colleagues studied 65 000 registered nurses aged 33 to 51 who reported in 1997 how much leisure-time physical activity they had done since the age of 12.

After six years of follow-up, 550 of the women were diagnosed with breast cancer.

Running or walking
The women classified as most active did the equivalent of running for 3.25 hours a week or walking for 13 hours a week.

"It's not marathon running. Any team sport will get you to that level of activity – and it doesn't even have to be a team sport," Colditz said.

Women who exercised regularly – but not as much as the most-active group – also had a reduced risk for breast cancer, but not as much as the top group, Colditz said.

The benefit provided by exercise was not associated with a particular sport or intensity, Colditz added.

About a quarter of all breast cancer cases are diagnosed in women before menopause and these can be more aggressive and harder to treat than breast cancer in older women.

Protection against breast cancer is just one of many benefits of exercise, Colditz said. "It protects against diabetes, heart disease, stroke. Clearly it's good for bones to protect against osteoporosis and fractures. There is a clear benefit across many of the chronic diseases."

He said there are several hormone-related hypotheses to explain how exercise may cut breast cancer risk. A leading idea is that physical activity can cut a woman's lifetime exposure to estrogen, a hormone strongly implicated in breast cancer.

Breast cancer is the leading cause of cancer death among women worldwide, killing an estimated 465 000 women annually, according to the American Cancer Society. About 1.3 million women are diagnosed annually worldwide.

as those who have children late in life, for example after the age of 30.

10. Breastfeeding reduces your risk.
Breast is best not only for the health of your baby. Studies have shown that women who breastfeed their babies have a lower risk of developing breast cancer later in life.

11. Starting periods young or having a late menopause can also increase your risk.
Onset of the menstrual cycle prior to the age of 12 and menopause after 50, increases the risk of developing breast cancer. This is because high amounts of oestrogen are in your body for longer.

12. Breast cancer is a mystery.
While experts have been able to pinpoint some risk factors, these only account for about 30% of all known causes of breast cancer. The remaining 70% are an unexplained mystery.

13. Breast cancer is many diseases.
What experts once thought was one disease is actually multiple diseases. The good news is that a greater understanding of breast cancer is slowly helping medical doctors to predict who needs more and who needs less treatment.

14. The key to breast cancer survival is early detection, so examine your breasts.
The sooner breast cancer is diagnosed, the better your chances of beating it, so learn to do breast self examinations every month a week after your period.

15. You need to go for regular medical check-ups.

If a mammogram shows a lump that is concerning, you will be referred for a biopsy. A needle is used to take a sample from the lump which is sent to a lab to see if it contains cancer cells. Although scary, it's a painless procedure.

21. Mammograms can detect breast cancer up to two years before it is large enough to be felt.
But be warned, this is not a comfortable experience! Your breast is flattened like a pancake between two plates so that X-rays can be taken of the tissue. Technique can help to make it more comfortable, so try and visit a specialist centre.

22. Breast cancer is not usually painful.
Most women are not very good about self examination and comfort themselves that a breast cancer lump would hurt, and this is not always so. Early breast cancer shows no symptoms and is not painful, which is why it's important to self examine and have regular check-ups.

23. A lump is not the only symptom of breast cancer.
Lumps are not always bad news and maybe due to a blocked milk duct or a sebaceous cyst, but they should always be checked. Other symptoms include a change in the shape or position of the breast, puckering of the skin which can invert the nipple, a rash on the nipple or discharge from it.

24. Myth: Finding a lump in your breast means breast cancer.
The happy news is that eight out of ten lumps are benign (not cancerous.) Even so every lump should be checked by a physician immediately.

25. Fear should not keep you from checking the state of your health.
Sometimes women avoid having a medical examination because they fear what they might find. But ignorance is not bliss and neither is living in fear. Being vigilant can literally save your life.

26. Treatment can be varied.
Treatment may include surgery, such as a lumpectomy, where the lump is surgically removed, a partial mastectomy which removes part of the breast, or a radical mastectomy, which removes the breast and lymph glands under the arm. This may be accompanied by chemotherapy (anti-cancer drugs taken in pills or intravenously in a drip) or radiotherapy. Radiotherapy uses electromagnetic currents or waves to penetrate a tumour and kill its cells. Chemotherapy uses a cocktail of drugs to attack and kill tumour cells.

27. Breast cancer can be fatal.
Over 3 000 South African women die from breast cancer each year, but many survive and become role models for other women and other cancer patients.

28. Breast cancer is not your fault.
Some people believe that breast cancer is a punishment, due to witchcraft or past sexual behaviour. Cancer is no one's fault.

29. More people are talking about breast cancer.
Twenty years ago it was taboo to talk about breast cancer. With more high profile people getting breast cancer, more people are able to talk about it and this increases awareness.

30. Know and touch your body from puberty.
In many cultures, it is still difficult to talk about your reproductive parts including your breasts, let alone touch them. We must teach our daughters and nieces to know their bodies and feel their breasts and check them every month from puberty.

31. You cannot catch breast cancer or transfer it to someone else's body.
Breast cancer is the result of uncontrolled cell growth which starts in the breast and cannot be passed on to another person.

32. Two genes, BRCA 1 and BRCA 2, have been linked to hereditary breast cancer,
and women in families that have mutations of these genes have a much higher risk of developing breast cancer than women who do not. However not all people who inherit these gene mutations will develop breast cancer.

33. Is it in your family?
Breast cancer is seen to be hereditary in only five to 10 percent of cases. To link cancer to your family, an immediate relative needs to have had it and it should be linked to more than three generations.

34. Breast cancer has plagued women for centuries.
Breast cancer is one of the oldest known cancer tumours. Even the ancient Egyptians knew that breast lumps could be dangerous and should be removed. References to breast tumours or ulcers have been recorded on papyrus which dates back to 1600 BC.

35. Myth: mammogram can cause breast cancer to spread.
No matter what you may have been told, a mammogram X-ray and the pressure of its machinery on the breast cannot cause cancer to spread.

36. A woman who exercises four hours a week reduces her risk of breast cancer.
Exercise pumps up the immune system and cuts your levels of the hormone oestrogen, which can fuel tumour growth.

37. A high-fat diet increases the risk of breast cancer
because fat also triggers the oestrogen. Fill your diet with plenty of fruits and vegetables and low-fat nutritious foods.

38. Smoking increases your risk of cervical and other cancers.
Although smoking has not been directly linked to breast cancer, smoking decreases your survival chances once you have been diagnosed with breast cancer.

39. Go easy on animal protein.
Those with a diet high in animal protein have an increased risk of developing breast cancer.

40. Breast cancer is not exclusively a women's disease.
For every 100 women with breast cancer, one man will develop the disease. Due to the stigma, it is difficult for men to talk about it or to go for medical treatment.

41. Humans are not the only mammals prone to breast cancer.

42. Myth: there is a link between the Pill and breast cancer.
There have been many studies done on the relationship between the oral contraceptive pill use and breast cancer, but these have failed to show that there is any real link. But it is used to delay pregnancy, which increases the breast cancer risk. There is a slightly increased risk of breast cancer for women who have taken birth control pills uninterrupted for long periods, such as five to ten years.

43. Hormone Replacement Therapy.
Women who suffer greatly from the side-effects of menopause often are prescribed hormone replacement therapy. However this does increase your risk of breast cancer.

44. Women in the West are more susceptible to breast cancer.
In Western societies 60 to 70 women for every 100 000 develop breast cancer, whereas 25 for every 100 000 Asian women do so. However when Japanese or Chinese women move to the USA or Australia for example, the rate rises to that of the West within two generations. This suggests an environmental influence, such as a high fat diet or alcohol use.

45. Alcohol plays a role in increasing risk.
More than two alcohol units a day increases your risk by 24%.

46. Being overweight or obese increases your risk.
Women who are overweight have an increased risk of cancer, especially breast cancer. This is due to the possible increased levels of oestrogen in overweight women.

47. Drink lots of water and avoid alcohol.
Drink plenty of fresh water each day to help flush out toxins and limit your alcohol intake to no more than one drink a day.

48. Exercise is a winner for women getting over breast cancer.
Women who do aerobic exercise and resistance training enjoy better fitness and quality of life than their more sedentary counterparts, a new study at a Spanish University has shown. Previous studies had shown that cancer survivors can become stronger and less fatigued with aerobic exercise, but now it's been proven that resistance training – like chest and shoulder presses, stomach crunches and leg and arm curls is very beneficial too.

49. Did you know that breast cancer is the second leading cause of cancer-related death among SA black women, after cervical cancer and is the leading cancer killer among white women.

50. Tamoxifen, the pill that can prevent breast cancer in high-risk women, does not appear to save many lives in the long run,
according to researchers from the University of California. Tamoxifen, which is the only approved drug for preventing breast cancer in women who haven't reached menopause, blocks oestrogen which can fuel the growth of tumours in some cases. But for women at the low end of the high risk group, the side effects outweigh the benefits as it can cause blood clots and uterine cancer.

Alcohol and Breast Cancer Risk

While the potential health benefits of moderate alcohol consumption have garnered a lot of public attention, alcohol's impact on cancer risk has received much less. Epidemiological studies have consistently found that heavy drinking can increase the risk of liver, head and neck, and esophageal cancers, and even moderate drinking has been shown to increase the risk of breast cancer.

At the 2008 American Association for Cancer Research (AACR) annual meeting in San Diego, two new studies were presented that shed additional light on the alcohol-breast cancer connection, including one study that linked alcohol consumption with a significantly increased risk of the most common type of breast cancer.

Even though these studies grabbed headlines, researchers stress that important questions remain unanswered, such as which women who drink are at greatest risk, and what biological mechanism(s) alcohol might trigger to cause breast cancer. In short, researchers are still accumulating evidence that can form the basis for personalized clinical recommendations.

Nevertheless, some recommendations have already been made. As part of a far larger report on cancer prevention released last year, a consensus panel formed by the American Institute for Cancer Research (AICR) concluded: "The evidence on cancer justifies a recommendation not to drink alcoholic drinks."

The AICR report also acknowledged, however, the consistent findings that moderate alcohol consumption can protect against heart disease, and offered that, if individuals choose to drink, women should limit their consumption to one alcoholic beverage per day and men to two.

But even a highly consistent association between alcohol intake and breast cancer risk "is not the same as saying causality has been proven," says Dr. Arthur Schatzkin, chief of the Nutritional Epidemiology Branch in the National Cancer Institute's (NCI) Division of Cancer Epidemiology and Genetics. The same, he adds, holds true for the protection against heart disease.

"The breast cancer risks involved with alcohol are indeed modest; nothing like the magnitude of the risks between smoking and lung cancer or HPV and cervical cancer," Dr. Schatzkin continues. "So it's difficult to be absolutely certain from the available studies that it's not some other biologic or behavioral factors associated with moderate drinking that are the real etiologic agents in breast cancer."

The important point, he stresses, is that "Drinking alcohol is an entirely avoidable risk factor," especially for women with established risks like a family history of breast cancer.

Studies dating back to the 1920s show that alcohol consumption and mortality risk are represented by a J-shaped curve: Risk of death is somewhat elevated in teetotalers, dips for moderate drinkers, and then climbs steadily as consumption increases.

According to long-term studies performed by Dr. Arthur Klatsky and colleagues at Kaiser Permanente in Oakland, CA, the vast bulk of the benefit of light-to-moderate alcohol consumption is due to an apparent protective effect against cardiovascular disease, primarily in middle-aged people.

Incidence data presented by NCI researchers at the AACR meeting were somewhat consistent with a J-curve, at least in terms of excessive alcohol consumption. Based on an analysis of more than 180,000 women in the NIH-AARP Diet and Health Study, they found that women who consumed three or more alcoholic drinks a day had more than a 50-percent increased risk of ER+/PR+ breast cancer, while women who drank smaller amounts also had an elevated risk, regardless of alcohol type.

The results, Dr. Klatsky notes, are mostly consistent with data from his studies and support the hypothesis that alcohol may increase breast cancer risk via an effect on estrogen. However, the results are not entirely consistent and highlight the difficulty in establishing a risk "threshold," Dr. Klatsky explains.

"Our data show that women who report having just several drinks a week don't have an increased [breast cancer] risk, and the risk begins somewhere between that and two drinks per day," he says.

In addition to the interplay between alcohol and estrogen, research has focused on several genes that code for the enzyme alcohol dehydrogenase (ADH), which is involved in alcohol metabolism. ADH initiates the breakdown of alcohol into acetaldehyde, ethanol's first metabolite, which is carcinogenic in animal models.

At the AACR meeting, researchers from Georgetown University's Lombardi Comprehensive Cancer Center and the State University of New York at Buffalo, using data from the Western New York Exposure and Breast Cancer Study, reported finding an increased breast cancer risk among postmenopausalwomen who drank and had variations in a gene that codes for ADH. The more the women reported drinking, the greater their risk.

"This is what we're really trying to get at now," says Lombardi's Deputy Director, Dr. Peter Shields, who co-led the study. "We're assuming that there are certain genetic susceptibilities. There's some evidence for it, but not enough studies to say that, for women who drink, certain genes put you at increased risk of breast cancer."

But other molecular players may be at work. Dr. Shields' lab has received funding from the Department of Defense to take a more systematic look at four potential causal mechanisms suggested by previous studies. These include the alcohol-estrogen link and the role of acetaldehyde, as well as alcohol-induced oxidative damage and disruption of folic acid pathways.

"We want to take this type of beverage that many women are going to drink," Dr. Shields says, "and figure out when they are really putting themselves at risk.

NEW YORK DECLARES SEPTEMBER 26 MESOTHELIOMA AWARENESS DAY

New York Governor David Patterson has declared Saturday, September 26, Mesothelioma Awareness Day. Mesothelioma, a rare form of cancer caused by exposure to asbestos, affects between 2500 and 3000 Americans every year.

The first Mesothelioma Awareness Day was held in Pittsburgh, Pennsylvania, in 2003. With the amount of industrial pollutants generated by the area’s steel mills, coal mines and other plants and factories, and the attendant lung diseases that many of the workers in those facilities would later develop, a group approached the Pittsburgh City Council and requested that they recognize Mesothelioma Awareness Day to help let other workers know about the dangerous disease and raise funds to finance treatments and clinical trials for patients. The group chose September 26 to commemorate the wedding anniversary of a patient who died from mesothelioma and his widow, who brought the cause to their attention.

In New Jersey, State Senator Tom Kean has proposed that his state recognize Mesothelioma Awareness Day in an effort to make sure that lawmakers, cancer researchers and the general public remember patients who have contracted the disease. In Kentucky, the state House of Representatives issued a proclamation to recognize Mesothelioma Awareness Day and to honor former state legislator Ron Cyrus, who died from the disease.
